None of this is required. If you just want the underfoot flag you can do the hotzone tasks or any of the group tasks.
I realize there were raids back in the day that had ridiculous numbers of people in a giant zerg fest but a) they weren't fun, B) they completely removed the difficulty of the encounter, C) they generally slowed the zone down to a crawl and frequently crashed it. The cap was set to strike a balance between maintaining the old feel of the raids and give some level of difficulty in their new incarnations. Retuning a nagafen for these events at a level of 54 people wasn't feasible from a time standpoint, and if you retune him it defeats the purpose of recreating the original encounter.
Underfoot has a couple of Monster missions, but it is not the focus by any means.
