Well, actually, for multi-boxers the biggest problem with auto follow is that it is client-side. All the following code is happening on the client. So if your background client is being starved of CPU it gets very confused. If you go into Options->Display->Advanced change the Max Background FPS to something other than "Min CPU" and it will work much better. Min CPU pretty much starves the background client.
